How the use-by time is calculated

Use-by = the moment the clock started + the window published for that storage location by the guidance body you choose. For freshly expressed milk the clock starts when it was expressed. For milk that has been thawed, warmed, or fed from, the clock restarts at that moment — not at expressing. That is the single most misunderstood rule in milk storage.

Freshly expressed milk

  • Room temperature. NHS: 6 hours. CDC: 4 hours at 25 °C (77 °F) or colder. ABM Protocol #8: 4 hours optimal at 16–29 °C, 6–8 hours acceptable under very clean conditions. NHMRC (Australia): 6–8 hours at 26 °C or lower.
  • Cool bag with frozen ice packs. NHS, CDC and ABM: 24 hours. The NHMRC table has no cool-bag row.
  • Fridge. NHS: up to 8 days at 4 °C or lower, or 3 days if the fridge is warmer or you are not sure. CDC: 4 days. ABM: 4 days optimal, 5–8 days acceptable under very clean conditions. NHMRC: no more than 72 hours at 5 °C or lower.
  • Ice compartment inside a fridge. NHS: about 2 weeks. NHMRC: 2 weeks at about −15 °C. CDC and ABM publish no separate figure, because that compartment rarely reaches −18 °C.
  • Freezer with its own door. NHS: 6 months at −18 °C or lower. CDC and ABM: 6 months best or optimal, up to 12 months acceptable. NHMRC: 3 months at about −18 °C.
  • Chest or deep freezer. CDC and ABM: 6–12 months. NHMRC: 6–12 months at about −20 °C. NHS publishes a single 6-month freezer figure.

Milk that is no longer fresh

  • Thawed in the fridge. CDC, ABM and NHMRC: use within 24 hours, counted from when it is completely thawed. NHS advises using it straight away once fully defrosted.
  • Thawed in warm water, or brought to room temperature. CDC and ABM: 1–2 hours. NHMRC: for the current feed, held a maximum of 4 hours until the next one. NHS: use straight away.
  • Warmed but not yet offered. CDC and ABM: 2 hours. NHMRC: 4 hours. NHS advises against re-warming at all.
  • Milk a baby has fed from. NHS: within 1 hour, then throw it away. CDC: within 2 hours of the feed ending. ABM: 1–2 hours. NHMRC: discard once the feed is over.
  • Re-freezing. Never re-freeze milk that has fully thawed. CDC allows one exception: if ice crystals are still visible it has not fully thawed and may be re-frozen.

Moving milk somewhere colder does not reset the clock. The window runs from when the clock started, so milk that has sat out for four hours does not get a fresh four days when it reaches the fridge. Freeze in 60–120 ml portions, leave headroom because milk expands, and store at the back rather than in the door.

There is no single answer, and that is the useful part

Search how long expressed breast milk keeps in the fridge and you will get four hours, six hours, three days, four days, five days and eight days, usually with no indication of where any of them came from. All of those numbers are real. They come from different national bodies weighing the same evidence with different safety margins.

The fridge is where the disagreement is widest. The NHS says freshly expressed milk keeps for up to 8 days at 4 °C or lower. The CDC says 4 days. Australia's NHMRC says no more than 72 hours. That is a spread of nearly three to one on the most common storage question there is, and if you have moved country, married across one, or read two websites in a row, this is why the advice you have been given does not match.

The Academy of Breastfeeding Medicine, whose Protocol #8 many services around the world follow, does something helpful with this: it publishes two figures. Four days in the fridge is optimal; five to eight days is acceptable under very clean conditions. That framing explains the whole disagreement. Nothing dramatic happens to milk on day five. The shorter figures build in room for real kitchens, warm fridges, and hands that were not washed as well as they might have been.

Room temperature and cool bags

At room temperature the range is narrower but still real: 4 hours from the CDC, 6 hours across NHS services, and 6 to 8 hours from the NHMRC at 26 °C or lower. ABM gives 4 hours as optimal and 6 to 8 as acceptable when conditions are very clean. Warmer rooms shorten all of these, and none of the bodies intends the upper figure as a target.

Cool bags are the one place almost everyone agrees: 24 hours with frozen ice packs, said by the NHS, the CDC and the ABM alike. Use frozen gel packs rather than loose ice, and keep the bag shut — the 24 hours assumes it stays cold, not that it was cold when you left the house.

Freezers are not all the same freezer

This is where guidance that looks contradictory is actually just more precise. The NHMRC splits freezers into three: the ice compartment inside a fridge at around −15 °C gives 2 weeks; a freezer section with its own door at around −18 °C gives 3 months; a deep freeze at around −20 °C gives 6 to 12 months. The NHS gives 2 weeks for the ice compartment and 6 months for a freezer at −18 °C or lower. The CDC and ABM give 6 months as best and up to 12 months as acceptable, and do not break out the small ice box at all, because it rarely holds −18 °C.

So if your milk is in the little compartment inside the fridge door, two weeks is the number, whichever country you are in. Freeze in small portions of 60 to 120 ml so you thaw only what you need, leave a gap at the top of the container because milk expands, and store it at the back where the temperature is stable rather than in the door.

The three rules that catch people out

Milk a baby has already fed from. Bacteria travel from the baby's mouth back down the teat into the milk, so this clock starts when the feed ends, not when the milk was expressed. The NHS is the strictest: use within 1 hour and throw away what is left. The CDC and ABM allow 2 hours. The NHMRC table gives no holding time at all — discard once the feed is over. A part-drunk bottle cannot go back in the fridge for later and cannot be frozen.

Milk that has been warmed. Warming starts the same short clock whether or not the baby ever touched the bottle, because it lets any bacteria present begin multiplying and re-chilling does not undo that. The CDC and ABM give 2 hours from warming; the NHMRC gives 4; the NHS advises against re-warming at all and treats warmed milk as being for that feed. Warm only what you expect to be taken. Never use a microwave: it heats unevenly, leaves hot spots that can scald, and damages some of the immune components in the milk. Stand the container in warm water instead.

Thawed but not re-frozen. Once milk has completely thawed it can never go back in the freezer. The CDC allows a single narrow exception: if ice crystals are still visible it has not fully thawed and may be re-frozen. Once they are gone the only options are the fridge, the baby, or the sink. The 24-hour fridge window for thawed milk is counted from when it is fully thawed — not from when you took it out of the freezer, and not from when it was expressed.

Which figure should you follow?

Follow the one your own health service publishes. If you are in the UK that is the NHS; in the United States, the CDC; in Australia, the NHMRC. If your baby is preterm or in hospital, none of this applies: neonatal units run stricter and shorter rules of their own, and their instructions come first.

Beyond that, the sensible habits are the same everywhere. Wash your hands and sterilise the equipment. Label every container with the date and time. Chill milk as soon as you reasonably can after expressing, because moving it somewhere colder later does not give it back the hours it has already spent warm. And if milk smells sour or looks wrong, throw it away regardless of what the table says.
