High Lipase Breast Milk: Why It Smells Soapy

A soapy, metallic or sour smell in stored milk usually comes from lipase, an enzyme that breaks down fats during storage. The milk is not spoiled and is safe. The Academy of Breastfeeding Medicine advises against heating milk above 40C to deactivate lipase, because that destroys many of the milk's protective factors.

What is happening in the bottle

Breast milk contains lipase, an enzyme whose job is to break down the fat in milk so a baby can digest it. It keeps working after the milk leaves you. The Academy of Breastfeeding Medicine's storage protocol explains the chain: refrigerated and frozen human milk may smell different from fresh milk because lipase breaks down triglycerides and releases free fatty acids, and the smell comes from those fatty acids oxidising. People describe it as soapy, metallic, fishy, like old cream, or like a wet tin. The HSE notes the same thing more briefly — refrigerated and frozen milk may have a different odour and colour from fresh milk, and most babies drink it regardless.

Lipase activity varies a lot between people. Some never notice it. Some find milk smells within a few hours in the fridge. Neither is a problem with your milk.

It is not spoiled, and it is not your diet

This is the important reassurance. The change is enzymatic, not bacterial. ABM Protocol #8 goes further and notes that the lipolysis process actually has antimicrobial effects, preventing the growth of microorganisms in thawed refrigerated milk. There is no evidence that infants often reject milk because of this odour, and the protocol makes the sensible point that plenty of foods humans eat — eggs, cheese, fish — smell unpleasant without tasting bad.

Nothing you ate causes it, and nothing you can eat will stop it. It is not a sign of poor hygiene, a dirty pump or a fridge problem.

How to tell it apart from milk that has actually gone off

  • High lipase. Smell is soapy, metallic or like old fat. It develops predictably over hours or days in every batch. The milk still looks normal when swirled.
  • Spoiled. Smell is genuinely sour or rancid, like off cow's milk. It happens when milk has been stored too long or too warm, and it does not follow the same predictable pattern.

If you are unsure, taste a drop. It is your own milk and there is nothing unsafe about it. Soapy is lipase; sharply sour is not.

Scalding: what it does, and what it costs

The widely shared solution is to heat freshly expressed milk to just below boiling to deactivate the lipase, then cool it quickly and freeze it. It does work on the smell. It also has a cost, and the guidance is clear about it. ABM Protocol #8 states that heating milk above 40C to deactivate lipase is not advised, because this may destroy many of the immunologically active factors in human milk. The HSE's separate advice on heating milk says not to let the temperature go above 40C and never to heat milk in a microwave or saucepan, because heating milk until it is hot can destroy some of the good properties in it.

So this is a genuine trade-off rather than a trick with no downside. Scalded milk is still milk, and milk your baby will drink is better than milk that gets poured away — but it is not a free lunch, and you should know what you are giving up. It is not something to do routinely to milk your baby is drinking happily.

What to try before scalding

  • Use it fresh or from the fridge. The smell builds with time, so milk used the same day usually has no problem at all. Save the freezer for milk you have tested.
  • Test one bag first. Freeze a small amount, thaw it after a few days and smell it. There is no point processing an entire stash if your baby will take it.
  • Offer it anyway. Most babies do not care. The reaction of the adult smelling it is not a good predictor.
  • Mix it. Combine a small amount of the affected milk with a larger amount of fresh milk, once both are chilled.
  • Serve it cooler. Cold milk smells and tastes less strongly than warm milk. The HSE says healthy full-term and older babies can drink milk straight from the fridge.
  • Chill fast. Getting milk into the fridge or freezer quickly after expressing slows the process down.

Freezing temperature makes a difference

ABM Protocol #8 cites a study showing that freezing milk to -80C leads to less change in smell than conventional freezing at around -19C. That is a hospital freezer, not a domestic one, so it is not a practical fix — but it does confirm that the effect is temperature-dependent and that a colder, more stable freezer is better than a frost-free one that cycles.

If your baby refuses the milk

Some babies do refuse strongly smelling milk, usually older babies rather than newborns. If that happens, work through the list above before deciding a whole freezer stash is unusable. If it is genuinely unusable, that is a horrible thing to discover and worth saying out loud: it is not a failure of storage or of you. Milk banks cannot usually take home-stored milk, but your health visitor, public health nurse or a breastfeeding helpline can help you plan around it.

What high lipase does not affect

It does not make your milk less nutritious while it is fresh, it does not affect feeding at the breast, and it does not mean your baby is getting anything different when they feed directly. It is purely a storage phenomenon. Nor does it predict anything about future babies, though people who have it with one often have it again.

If you are donating milk

Milk banks have their own screening, handling and pasteurisation processes, and they set their own requirements for how milk is expressed and stored. If you are donating, follow the bank's protocol rather than adapting anything from home practice, and tell them if your milk develops a strong smell, so they can advise.

When to stop assuming it is lipase

If milk smells sour rather than soapy, if your baby refuses it and also refuses fresh milk, or if they are unsettled after feeds of stored milk in a way they are not after feeding at the breast, stop troubleshooting the smell and speak to your health visitor, public health nurse or an infant feeding team.
